A full day in a single city leaves real time to go inside properly rather than admire things from the pavement, and if you want to add a second stop your guide will tell you what it costs and how long it takes.
Not as part of the tour, but the jetties sit right in the centre and your guide can point you to the nearest one if you would like to add a boat trip. Tickets are paid on the spot.
There is a fair amount, almost all of it on cobbles, though Bruges is flat and compact. Because the tour is private, your guide sets the pace around you and you stop whenever you like.
